Your Appointment

To provide you with the best care possible, there are a number of forms we ask you to complete prior to your first appointment. These can be completed on the day of your appointment, or you can download this form and complete it prior to coming to see us.

Please bring the following to your first appointment:

  • A referral letter from your GP

  • Your Medicare card and health care card

  • Health insurance details

  • Any blood tests or imaging (including ultrasounds) you have had done

  • Information regarding any operations or procedures you have had

  • List of medications

  • Any questions you might have

During an initial appointment, Dr Melissa will discuss your symptoms and concerns. A physical examination will also be performed, which may also include a pelvic examination. Dr Melissa will then discuss any further investigations that are required, as well as possible treatment options.

Cancellations

We understand that circumstances change, and you may need to change or cancel your appointment. We ask that you inform us at least 24 hours prior to your appointment, as this allows us to offer your appointment time to someone else.

Payment

This is a private practice clinic where payment is required on the day of your consultation. A portion of your consultation fees will be covered through Medicare. For surgical procedures, Dr Melissa offers Known Gap for eligible health funds.

We will provide you with a quote prior to your surgery. Please check with your health fund that you are covered for your procedure prior to your operation.

Payments can be made with cash, EFTPOS and major credit cards.
